 HOW TO APPLY

INSTRUCTIONS FOR ONLINE APPLICATION

  1. Candidates are required to apply online through website: http://cpdhe.du.ac.in/apply-online.php.
  2. Before filling the online application, please read the instructions carefully.
  3. Entries in the application form shall be required to be filled only in English.
  4. Candidates are advised to use a valid Email ID and Mobile Number for future reference.
  5. Separate application shall be submitted for each Orientation Programme / Refresher Course / Short Term Course / Workshop.
  6. Once the information filled, no change will be accepted after that.
  7. Before starting to fill up the online application, keep ready with you the following documents: (a) Image of scanned photograph in jpg / jpeg format and (b) Image of scanned NOC in jpg / jpeg / pdf format. Click to download No Objection Certificate (NOC).
  8. Click on APPLY ONLINE to fill the application form.
  9. Fill the details on the online application form, upload Photograph and NOC Format (duly signed by the head of organization with official seal).
  10. To submit the application click Apply Now.
  11. Candidates applying online are required to take print-out of auto generated filled in application.
  12. To complete the registration process please send the print-out of the signed application form and original NOC in a sealed envelope superscribed “Application for _____________” on the top of the envelope, through ordinary / speed post, to the address given below:

  1. Application shall reach CPDHE Office within 15 days from the date of applying Online.
  2. CPDHE shall not be responsible for any delay / loss due to postal or technical reasons.
  3. Incomplete applications (without prescribed NOC and photograph) shall be treated as cancelled.
 
INSTRUCTION FOR OFFLINE APPLICATION
Download offline Application Form, fill it and get it forwarded by the Head of the Institution and send it on the above-mentioned address (follow step number 12 only).
